Job Title: Chronic Care Management Care Coach
Classification: 1099 Contractor
Work Structure: Fully Remote
Schedule/Shift: 25 -40 hours/week *(Pacific Time Hours)
Team: Clinical Operations
Reporting to: Head of Clinical Operations
Location: United States, Pacific Time Zone
Compensation: $30/hour

Position Summary
The Chronic Care Management Care Coach at Recora is responsible for providing care management services to patients with multiple chronic conditions in a remote setting. This role involves working closely with healthcare providers, patients, and their families to ensure effective management and coordination of healthcare services. The Care Coach will focus on enhancing patient engagement, adherence to care plans, and achieving clinical objectives specific to chronic conditions and preventive health measures.
Key Responsibilities
Patient Management:
- Conduct telephonic and virtual care management for patients with chronic diseases such as heart disease.
- Review and assess patients' medical, functional, and psychosocial needs remotely.
- Perform medication reconciliation and ensure adherence to prescribed therapies.
- Facilitate timely access to preventive services and reinforce disease self-management education.
- Coordinate care transitions between healthcare providers and settings.
- Manage a roster of assigned patients each month, ensuring all care management activities are completed in a timely manner.
Care Coordination:
- Collaborate with providers, specialists, and community resources to optimize patient care.
- Schedule and manage follow-up appointments, tests, and treatments.
- Notify patients of lab results and changes in care plans.
- Assist in managing care transitions and reducing hospital readmissions.
Patient Engagement:
- Educate patients and families about chronic disease management and available community resources.
- Support patients in self-management and behavior modification interventions.
- Conduct motivational interviewing to encourage lifestyle changes.
Documentation and Compliance:
- Maintain accurate and timely documentation in electronic health records (EHR).
- Ensure compliance with Chronic Care Management program requirements and protocols.
- Adhere to all organizational policies, including confidentiality and data security protocols.
Qualifications
Education:
- Bachelor's degree in Nursing, Social Work, or a related healthcare field.
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred.
- Current CPR certification.
Experience:
- Minimum of 2 years of experience in chronic care management or a related healthcare setting.
- Experience working with patients with complex chronic conditions is preferred.
Skills:
- Strong knowledge of chronic disease states and management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a healthcare team.
- Proficiency in using electronic health records and other healthcare technology.
Work Environment
- This is a fully remote position, allowing flexibility in work location.
- Must have a reliable internet connection and a suitable home office environment.
